• All accounts are established under NET 15 payment terms.
•  Payment is due on the 15th of the following month for all services rendered during the previous month. 
• A two-week grace period is provided for payment processing.
• Any payment received after the 30th of the month will be subject to a 5% late fee per invoice.
• The office agrees to maintain timely payments in accordance with these terms.

Straight Wire Orthodontic Laboratory provides a 60-day warranty on appliances covering manufacturing defects and workmanship issues.

The warranty does not cover appliances damaged or compromised due to:

• Patient negligence or misuse.
• Improper handling or care.
• Loss or accidental damage.
• Changes in the patient's dentition or treatment needs.

Warranty claims will be reviewed on a case-by-case basis. Appliances returned for warranty evaluation may be subject to inspection to determine whether the issue is related to manufacturing or workmanship.
